Your trusted advocate, care team, and partners in recovery,
Your bridge back to life after injury
Suffering a catastrophic injury is life-changing.
Following inpatient rehabilitation, the transition home and back to life can be just as challenging and very frightening.
Designer Health Partners was founded by Dr. Anna Elmers in 2020 with the belief that everyone needs an advocate and navigator through the very complex medical system.
Most importantly, DHP bridges the gap and provides hope after hospital discharge.

“When patients leave the very secure nest, it can be overwhelming and many of them continue to need care.”
Designer Health Partners exists as a bridge from the inpatient an outpatient rehabilitation setting to the client’s home setting.
